By reviewing these forms before we sit together, you are beginning the process of naming the narratives that have shaped your life.

As a Professor, I view our intake as a primary text. We move beyond medical checklists to map your geography, your lineage, and your relationship to power.

We recognize that naming your history and mapping your future can be heavy work. You do not have to do this in isolation.

II. Identity & Coverage Setup

To ensure the integrity of your sanctuary and the accuracy of your clinical record, we require a verification process. Once your PracticeBetter Client Portal account is active, please complete your profile by uploading the following into the “Documents” portion of your account:

  • A clear photo of your Government-Issued ID.
  • The Front and Back of your insurance card.

This verification ensures that our clinical sanctuary remains secure and your benefits are accurately applied.

III. Measuring Your Liberation: Greenspace

At Orange Oasis Healing, we utilize Greenspace to ground our work in Measure-Based Care. This is not about “scoring” your trauma; it is about tracking your evolution and ensuring our sessions are actually serving your liberation.

New clients are invited to establish their Greenspace accounts immediately. This platform allows us to:

  • Pivot our therapeutic approach based on real-time data from your lived experience.

How to Submit Your Testimony

While you are welcome to review the language of these forms via the links above, all official signatures and submissions are handled through our secure, HIPAA-compliant portal. We do not use paper forms or downloadable PDFs.

  1. Access the Portal: Once your consultation is confirmed, you will receive a secure invite to the PracticeBetter Client Portal.

  2.  Take Your Time: These are not forms to be rushed. These questions regarding your lineage, your relationship to power, and your goals for liberation require reflection. Find a quiet space, listen to your “felt sense,” and answer from your truth.

  3. Standalone Assessments: If you are visiting for a standalone Neuropsychological Assessment (ADHD/Autism/TBI), you will receive a specialized set of clinical history forms tailored to your diagnostic needs.

  4. Submit Digitally: Once completed, your forms are securely transmitted directly to me.
